Award Eligibility:

NATA member in good standing
At least 10 years BOC certification
Nominees must be nominated by a NATA member in good standing

Please provide details on how this athletic trainer has contributed in a meaningful way to the athletic training profession and/or athlete care above and beyond outstanding work in their day-to-day job including specific organizations, efforts or instances that exemplify his/her commitment. Please list charitable or civic causes that he/she has supported and his/her involvement. Please provide an example of a time that this individual has shown how genuinely he/she is concerned about his/her athletes and/or peers in athletic training.
Please explain how he/she has contributed to the betterment of the athletic training profession (i.e. served on an NATA standing committee, spent time to educate others about the profession, or actively advanced the NATA or athletic trainers in a specific way) for at least five years.
Please provide insight on this individual's character and specific examples of when his/her high integrity and unquestioned character has been proven.
